How is sexual dysfunction diagnosed?

Sexual dysfunction is any problem which occurs during any phase of the usual sexual response that prevents a person from engaging in or enjoying sexual activity. The typical pattern of the human sexual response happens in stages; excitement, plateau, orgasm, and resolution. Desire and arousal are part of the excitement stage, and these aspects are most commonly affected by sexual dysfunction. Research has found that sexual dysfunction is found in as many as 43% of women and 31% of men reporting some difficulty with desire and arousal. But these figures are mere estimates, as many people are reluctant to discuss such delicate issues.

Who is affected by sexual dysfunction?

Sexual dysfunction can affect people of any age, although it is more common in those over 40 years of age as it can be related to a decline in health and hormonal changes which are associated with aging.

What are the symptoms of sexual dysfunction?

In men:

  • Inability to achieve or maintain an erection, which is known as erectile dysfunction
  • Failure to ejaculate despite sexual stimulation, also known as retarded ejaculation
  • Failure to control the timing of ejaculation, known as premature ejaculation  

The specific medical criteria for erectile disorder are as follows:

In almost all or all sexual activity, at least one of the following symptoms: (1-3) and all of (4,5,6)

(1) marked difficulty in obtaining an erection during sexual activity,

(2) marked difficulty in maintaining an erection until the completion of sexual activity, or

(3) marked decrease in erectile rigidity

(4) The symptoms above have persisted for approximately 6 months

(5) The symptoms above cause significant distress to the individual

(6) The dysfunction cannot be better explained by nonsexual mental disorder, a medical condition, the effects of a drug or medication, or severe relationship distress or other significant stressors

Many prescription medications have been associated with ED:

  • Antihypertensive drugs
  • Antiulcer drugs
  • Lipid-lowering
  • 5-Alpha reductase inhibitors (eg, finasteride and dutasteride)
  • Antidepressants
  • Antipsychotic drugs (especially risperidone)
  • Testosterone and anabolic steroids

Premature (early) ejaculation generally occurs in men younger than 40 years and can put a great deal of stress on the couple’s relationship. A history of premature ejaculation can be seen in many men who present later in life with erectile difficulty. Effective treatments, including selective serotonin reuptake inhibitor (SSRI) medications and sex therapy, are available to remedy this condition

In both men and women:

In the IIEF-5 (seen below), the following 5 questions with respect to the preceding 6 months should be answered: Scored 0 to 5 with 5 being the highest.

  1. How do you rate your confidence that you could achieve and maintain an erection?
  2. When you had erections with sexual stimulation, how often were your erections hard enough for penetration?
  3. During sexual intercourse, how often were you able to maintain your erection after you had penetrated your partner?
  4. During sexual intercourse, how difficult was it to maintain your erection to the completion of intercourse?
  5. When you attempted sexual intercourse, how often was it satisfactory for you?

The answers to these 5 questions are each scored on a scale of 0-5. A score of 25 is typical for a healthy man; scores of 11 or lower indicate moderate-to-severe ED.

Causes of Sexual Dysfunction

Physical Causes

Many medical conditions can cause problems with normal sexual function. These conditions include diabetes, heart problems, neurological disorders, hormonal imbalances, kidney or liver failure, and alcoholism or drug abuse. The side effects of medications ordinarily taken for chronic conditions can also affect sexual function.

Psychological Causes

Stress and anxiety, concerns about sexual performance, marital or relationship problems, depression, being overly body-conscious, and past sexual trauma can all contribute to sexual dysfunction.

How is sexual dysfunction diagnosed?

In most cases, a person goes to see their doctor once they realize that their sexual dysfunction is causing problems within their relationship. Your doctor will usually take note of any symptoms and do a physical exam to see if it can be explained by an underlying health problem.

Then, a person’s psychological status will be considered. Any past trauma, abuse, relationship issues, medications, and substance abuse will be taken into account. The doctor will then be able to determine whether or not a referral to a specialist for treatment of any other problems is necessary.

How is sexual dysfunction treated?

Most types of sexual dysfunction can be corrected by treating any underlying physical or psychological issues. Other treatment options can include:


Where medication is the cause of sexual dysfunction, a change in the medication may help. Men and women with hormone deficiencies can benefit from being given hormone shots, pills, or creams. For men, drugs, including Viagra, vardenafil, and avanafil can help to improve sexual function by increasing blood flow to the penis.

Before the advent of the above-mentioned drugs, various other oral medications were investigated for treatment of ED, but are rarely used today:

  • Adrenergic receptor antagonists (eg, phentolamine, yohimbine, and delequamine)
  • Dopamine receptor antagonists (eg, apomorphine and bromocriptine)
  • Serotoninergic receptor activators (eg, trazodone)
  • Xanthine derivatives (eg, pentoxifylline)
  • Oxytocinergic receptor stimulators (eg, oxytocin)

Mechanical Aids

Aids such as vacuum devices and penile implants may help men with erectile dysfunction. A vacuum device called Eros is also approved for use in women, but it can be expensive. Vaginal dilators might help women who experience narrowing or tightening of the vaginal muscles.
